Triangle of Support
The fitness and/or nutrition specialist motivates the patient to health and wellness activities, and provides adherence to the patient during their transformation.
The Lifestyle Provider educates, sets goals, and connects patients to a specialist; as well as monitors outcomes and reports back to the physician for inclusion in the patient’s EHR.
The Physician writes the prescription for the patient to better their health and wellness. Provides oversight and monitoring of the patient during their transformation.
