Here’s a step-by-step guide to help you navigate this process smoothly:
Step 1: Notify Your Health Coach Care Coordinator (CC): First, inform your Health Coach or CC that you are considering ending your Care Plan. This can be done either over the phone or through our PACE mobile app messaging system. Your CC is your main point of contact and will guide you through the initial stages of this process.
Not getting along with your assigned Health Coach Care Coordinator (CC)? No problem! We understand every pairing might not be the perfect fit. You will not hurt your CC’s feelings by asking for a different health coach.
Step 2: Consult Your Referring Doctor: Next, it is crucial to discuss your decision with your referring doctor who prescribed our program and monitors your results on a regular basis. They can provide medical insights and support your decision with their professional opinion. Their input is invaluable in ensuring that any changes to your Care Plan are made with your orthopedic health and overall well-being in mind.
Step 3: Speak to a Lead Care Coordinator (CC) or RN Supervisor: After discussing with your referring doctor, the next step is to speak to a Lead CC or RN Supervisor. They will conduct a comprehensive review of your situation, taking into account various factors that influence your Care Plan. The Lead CC or Supervisor will then make a decision on whether to keep your Care Plan open or to proceed with ending it. This decision is not taken lightly and involves careful consideration of several factors, including:
