Message and data rates may apply. Message frequency will vary, and you will be able to Opt-out by replying “STOP”. Parts of our Site can be accessed without providing any Personal Information and other parts cannot. In order to access the Site anonymously, one would not voluntarily submit their Personal Information to us and continue using the Site as a guest. Our PACE application requires you enter some Personal Information to sign up and use the Site. You have the ability to refuse to supply any type of Personal Information although it may limit your ability, or access, to use our services and/or the Site.

Prescribe FIT collects this information through forms and through communications. Prescribe FIT will keep your data on file until you request, we delete your information. Depending upon how you use the Site, we may retain some Personal Information.

II.        How We Use and Share Information

Except as otherwise stated in this Privacy Policy, we do not sell, trade, rent or otherwise share for marketing purposes your Personal Information with third parties without your consent. We do share Personal Information with others who are performing services for Prescribe FIT. In general, the Personal Information you provide is used to help us communicate with you.

In general, we use Non-Personal Information to help us improve the Prescribe FIT Site and customize the user experience. We also aggregate Non-Personal Information and may use or provide Non-Personal Information to other parties for marketing, advertising, or other uses. The Site may include social media features such as the ability to share Site information on your social media site.  These features may collect your IP address, which page you are visiting on our sites, and may set a cookie to enable the feature to function properly. Social media features and widgets are either hosted by a third party or hosted directly on the Site. This Privacy Policy does not apply to these features. Your interactions with these features are governed by the privacy policy and other policies of the companies providing them.

1.  Terms

Certain health and medical information about you is protected under the Health Insurance Portability and Accountability Act (“HIPAA”) and applicable state laws. You may provide this information to Prescribe FIT, Inc. (“Prescribe FIT”) either online or offline, or Prescribe FIT may collect this information from other methods such as through a health care provider. We protect covered health and medical information as HIPAA and applicable state laws may require. Similarly, we may use covered health and medical information as HIPAA and applicable state laws permit. Your authorization is required as a condition of acceptance in the Prescribe FIT program. Prescribe FIT must have this information to administer its services. The individually identifiable health information provided to Prescribe FIT is not transferred, sold or otherwise disclosed to third parties, except as necessary for the program’s administration, or as may be otherwise required by law, and is always protected as Confidential Private Information.

2.  Authorization

I understand that by checking the box on the previous page, I agree that this constitutes my written authorization for Prescribe FIT to receive and use the individually identifiable health information described above for the proper administration Prescribe FIT’s program in accordance with applicable law. This authorization is voluntary and shall remain in effect for the duration of my participation in Prescribe FIT’s program. I understand that if the organization authorized to receive the information is not a health plan or health care provider, that the released information may no longer be protected by federal regulations. I also understand that the information used or disclosed pursuant to this authorization, except as prohibited by 42 CFR Part 2 or other applicable law, may be subject to re-disclosure by the recipient and may no longer be protected by HIPAA or state law. I understand that I may inspect or obtain a copy of the information to be used or disclosed. I understand that there may be a charge for copying my records. I understand that, if I have any questions about my health information’s disclosure, I can contact Prescribe FIT at tech@prescribefit.com. If you are agreeing on behalf of dependent family members, your agreement verifies that you are the parent, legal guardian, or the authorized representative of the individual identified on the enrollment form. I have the right to revoke this authorization in writing at any time by contacting Prescribe FIT, Inc. at 401 West Town Street, Suite 232, Columbus, Ohio 43215, except to the extent that my medical information has already been used or disclosed in reliance on this authorization. However, because this information is essential to this program’s administration, my revocation of this authorization shall result in cancellation of my participation in Prescribe FIT’s program.
