How to Take a Mobility Survey

Here is how to track your pain using the monthly Mobility Surveys in the PACE mobile app:

1) From the Homepage, scroll down to the “Mobility Survey” section

2) Read the descriptions on the five mobility survey questions and select the answer that mirrors your current state

3) Review your selection and click “Next”
